                        What if you are not ready for hospice care? Palliative care may be a good option for you if you have been diagnosed with a serious illness or disease such as congestive heart failure, cancer, kidney failure, dementia, Parkinson’s disease, or chronic obstructive pulmonary disease and you are not yet ready for end-of-life care. You many need additional help to manage pain and such symptoms as nausea, poor appetite, depression, and shortness of breath or anxiety.
Our Steward Center for Palliative Care is a nationally recognized model setting the “gold standard” for excellence in managing serious illnesses. The physicians and nurse practitioners provide home-based palliative care assessments and follow-up visits on a limited basis. They can assist with your care during hospital stays, and after discharge can offer outpatient services for continued help. The team can also aid in transitioning you to home health care services or hospice care when needed.
