 Personal Planning Checklist
Important Documents to Complete and Compile Now
ADVANCE DIRECTIVE
Fill out an advance healthcare directive. Complete a LaPOST (Louisiana Physician Order for Scope of Treatment) form with your doctor or hospice team member and post on refrigerator.
TRUST/WILL
Contact your attorney for advice. If you need help your Hospice Social Worker can assist you as well.
FUNERAL PLANNING
Contact the mortuary of your choice to finalize arrangements for services.
LIST OF KEY PEOPLE TO CONTACT
Phone numbers or email address for friends, colleagues, religious community members, and others you would like notified.
IMPORTANT PASSWORDS
Put passwords for ATMs, computers, phones, email, and online accounts in a safe place. Be sure that someone you trust knows where they are.
IMPORTANT DOCUMENTS TO GATHER
- Insurance policies
- Account locations and numbers for
important bills, loans, mortgages
- Complete list of all personal property
(real estate, stocks, bonds)
- Safe deposit box and key locations
- List of employer benefits
- Certificate of military discharge
if applicable
Records to Update After a Death
CREDIT CARDS
Call credit card customer service to cancel or change the name on accounts.
HEALTH INSURANCE
Contact your insurance provider to change or terminate coverage. Notify doctors and dentists to cancel future appointments.
VETERAN’S ADMINISTRATION
Contact the VA Benefits Department. You must have military service records when you contact them.
UTILITY COMPANIES
Electric, gas, water, telephone, cable,
cell phone: you may need to have the name changed or the services adjusted or terminated on all of these.
AUTO INSURANCE/REGISTRATION
Notify the auto insurance company for adjustments to premiums or cancellations. Contact DMV to transfer titles of vehicles.
LIFE INSURANCE/RETIREMENT BENEFITS
Contact your insurance company directly for filing claims and investigating benefits. Contact the Human Resources Department or Plan Admin for information on pensions, IRAs, 403(b), or 401(k) accounts.
BANK ACCOUNTS/SAFE DEPOSIT BOX/SECURITY BROKERS
Many financial institutions require a copy of the Trust/Will or Death Certificate to make account changes.
SOCIAL SECURITY
Generally the funeral home will contact Social Security for you, but you will need to contact them regarding any change of benefits or claims.
