 We Honor Veterans
The national based We Honor Veterans Program is a collaborative relationship between the National Hospice and Palliative Care Organization (NHPCO), the Department of Veteran Affairs (VA), and local hospice organizations. Guardian Hospice is a proud We Honor Veterans partner! This program is designed to build professional and organizational capacity to provide quality care for Veterans; develop and strengthen partnerships with VA and other Veteran organizations; increase access to hospice and palliative care for Veterans living in our communities; and provide opportunities to network and implement best practices models.
Veteran-Centric Care
Guardian Hospice Care and Volunteers are trained professionals receiving ongoing education regarding Veteran-specific issues at end of life.
Military History Checklist
Use of a Military Honors Checklist supports our ability to identify Veteran-specific issues/concerns and adapt the Veteran’s care plan to those needs.
Community Education
Guardian Hospice Care provides education to agencies, facilities, and families to ensure the needs of Veteran patients are met at end-of-life.
Veteran-to-Veteran Volunteers
Veteran Volunteers have the unique ability to relate and connect with each patient, creating an environment where life review and healing may occur.
Experts on Veteran Benefits
Veterans and their families receive support and expertise to navigate the complicated VA benefit structure and assist in the coordination of potential benefits for the Veteran and/or their family.
Military Honors
Guardian Hospice Care helps facilitate access for eligible Veterans:
→ Burial Flags
→ Headstone Markers
→ Flag Folding and Presentation
→ Playing of Taps
→ Veteran’s Cemetery Burial
Pinning Ceremonies
Veteran patients are given the opportunity to be recognized for their service through a presentation of a framed, branch-specific certificate and American flag pin.
